Pride Health is hiring for an RN Case Manager for a client in Kissimmee, FL! (Contract)
Schedule: Mon-Fri (8am-5pm)
Contract: 3 month contract
Pay Rate: $42-46/hour
Primary Responsibilities:
- Works with the providers and clinic staff to identify patients at high risk.
- Perform assessment of health conditions
- Perform medication reconciliation
- Conduct Motivational Interviewing and Self-Management Goal setting
- Provide patient education
- Create referrals to appropriate agencies and resources
- Supports transition of the patient with chronic care conditions from inpatient to outpatient setting
- Perform assessment of transitional needs
- Perform medication reconciliation
- Establish and reviewing contingency plan
- Provide patient education
- Assist with post discharge needs such as prescriptions, transportation, Durable Medical Equipment (DME), appointments
- Coordinate with providers to establish or update plan of care
- Performs accurate and timely documentation in the electronic medical record
- Participates in daily huddles and Patient Care Coordination (PCC) meetings
Required Qualifications:
- Active RN license
- Case management experience
- Certified Case Manager (CCM) is a plus
- Current BLS and/or CPR certification
- Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD
