Position Summary

We are seeking an RN Case Manager to provide exceptional home health care services while maintaining the flexibility to fit your life. This position is ideal for nurses who value work-life balance and want the flexibility to care for their families while managing patient care. Whether you're a parent with kids or someone who values being home on weekends, this role offers the perfect balance between professional and personal commitments.

In this role you will:Conduct assessments and complete client visitsCoordinate care alongside therapies & client/families and coordinate, supervise, monitor, and develop care. Communicate with physiciansPerform teaching to clients and families Complete complex nursing tasks within the home settingHandle case assignments that fluctuate, with typical caseload notifications by Friday.

Qualified candidates will have:

* Possess a valid RN License from the appropriate state. This shall include required CEU's

* Minimum 2 years of nursing experience

* Oasis experience (Preferred - will provide training)

* Home Health experience required
