Job Title: Case Manager RN Department: Care Management Status: Full-Time, Day Shift (Mon-Fri, 8:30A-4:30P or 9A-5P with monthly weekend rotation) Sign On Bonus: $5,000 Salary Range: $120,000 to 150,000 About the Role: As a Case Manager RN you'll play a vital role in coordinating care for patients with complex medical needs. You'll work closely with patients, families, and healthcare providers to ensure optimal outcomes and facilitate a smooth transition from hospital to home or other care settings. Key Responsibilities:
- Conduct comprehensive assessments to identify patient needs.
- Develop and implement individualized care plans.
- Coordinate with healthcare providers and community agencies.
- Educate patients and families about their care plan.
- Facilitate discharge planning and transitions of care.
- Advocate for patients' rights and interests.
- Contribute to quality improvement initiatives.
Qualifications:
- Bachelor's degree in Nursing (BSN) or related field.
- Minimum of five years of clinical experience in ED, critical care, or med/surg.
- Previous case management experience preferred.
- Knowledge of healthcare financial, regulatory, and payer issues.
- Knowledge of state, local, and federal programs.
- Experience with Milliman/InterQual criteria.
- Registered Nurse (RN) NYS license required.
- PRI/Screen certification strongly preferred.
- C.C.M. (Certified Case Manager) certification preferred.
- Strong computer skills.
