Job Title: Clinical Director - Home CareJob Type - Full Time/Direct HireLocation - Manhattan, NYJob Summary: We are seeking an experienced and compassionate RN Clinical Director to join our exceptional team of home health care providers. This full-time role is based at our Manhattan office but requires field supervision and family engagement with our adult and pediatric patients, including those on ventilators. The RN Clinical Director will ensure compliance with policies and procedures, oversee Private Duty Nursing (PDN) services, and foster a supportive environment for both patients and caregivers.Key Responsibilities
- Oversee the development, implementation, and revision of patient care plans in line with physician orders.
- Conduct in-person patient assessments, re-assessments, and quality assurance visits to ensure care standards are met.
- Assist in matching caregivers to patients based on clinical and psychosocial needs.
- Travel 1-2 days per week for on-site supervision and family interaction.
- Be available for on-call duties to manage urgent situations or provide support as needed.
Qualifications
- Current and valid NYS Registered Nurse (RN) license.
- Minimum of 1 year of experience in home care, conducting supervisory visits and patient assessments (Preferred).
- Familiarity with Private Duty Nursing (PDN) for adult and pediatric patients, including ventilator care.
- Schedule:
- Standard hours: Monday to Friday, 9 AM - 5 PM.
- On-call availability for patient emergencies or caregiver support.
- Weekends off (Saturday and Sunday).
