Job Description Responsible for the work flow coordination within Home Care Home Base often working week days, weekends or after hours. In addition, PCC must be able to do an assessment and evaluation of patients. The Patient Care Coordinator assures compliance with agency standards and state and federal laws

**General Responsibilities**

* Plans, coordinates and delivers care to patient and families as appropriate.

* Manages and coordinates work flow within Home Care Home Base.

* Participates in Administrative On Call.

* Delegates and assigns the patient/family to appropriate staff and team members, ensuring highest quality of care.

* Visits patients and families as needed.

* Maintains and facilitates interdisciplinary team/group meetings for continual updating of patient care plans, exchange of information and problem solving.

* Develops and promotes programs for continuing staff support, inservice and education.

* Monitors compliance with rules and regulations of Federal and State regulatory agencies and accreditation requirements, and policies & procedures of agency.

* Assures complete and accurate clinical documentation in compliance with all regulatory agencies.

* Interviews and provides orientation and training for the staff.

* Accepts other assignments deemed appropriate per the Senior/Director of Professional Services and/or Patient Care Manager

* Collaborates with service vender regarding appropriate cost effective services and accurate billing.

* Participates in on-call coverage.

* May serve on Safety Committee and/or QAPI Committee.

* Is knowledgeable of all Heartland services in order to foster referrals within the company.

* Able to perform functions of field staff. May be required to perform field assignments.

* Ensure growth & profitability of the company through the responsible use of company resources & educating thecommunity to our services.

* Willingly takes direction from Senior/Director of Professional Services and/or Patient Care Manager.

* Meets or exceeds established productivity standards.

* Adheres to established personnel policies.

Job Requirements **Education**

* Currently licensed as a Registered Nurse (RN) in state which he/she practices.

**Qualifications**

* Demonstrates problem solving and decision making abilities.

* Ability to manage patient care with specific knowledge and experience in bedside nursing, symptom management,

* Experience in crisis and family intervention.

* Demonstrated management ability with an interdisciplinary team and other health care providers.

* Excellent skills in:

+ communication both verbal and written

+ interpersonal relations

+ documentation

+ organization

* Ability to develop and maintain rapport with the patient, families, and staff.

**Experience**

* Minimum of one year experience in providing medical, oncology, home health care, or hospice nursing preferred, may be specified by state. Previous management or supervisory experience preferred.
