Compass Healthcare Consulting & Placement is conducting a search for an experienced RN Director of Case Management, for a Hospice located in the Bronx, NY. This is an opportunity to support quality care, working with a team of clinical staff and healthcare leaders to achieve optimal patient outcomes. Registered Nurse, RN with Director level Hospice/Hospital Case Management, experience required. Qualified candidates will be an experienced Clinician, RN with a Case Management and Utilization Management background, with an emphasis on Case Management and Director level experience.

Director of Case Management (Hospice) - RN

Oversees the daily operation of the Case Management Department which is responsible for the coordination of the hospital-wide functions of Case Management, Utilization Review and Discharge Planning. Oversees and coordinates the collection of pertinent Hospital data and provides educational activities to all levels of Hospital staff regarding the above functions.

Responsibilities:

  • Provide strategic and administrative oversight and leadership to the Case Management Department.
  • Develop, implement and evaluate case management programs, including utilization review, and discharge planning.
  • Establish and maintain relationships with community-based services and organizations for referrals.
  • Evaluate patient care data and documentation to ensure care provided in accordance with clinical guidelines and organizational standards.
  • Seek treatments that balance clinical and financial concerns with the family's needs and the patient's quality of life.
  • Contribute to the development and improvement of clinical pathways that enhance cost effectiveness while providing quality care.
  • Consult to optimize patient outcomes and length of stay metrics in alignment with best practices, hospital priorities, payor requirements and regulatory requirements.
  • Implement and monitor the Utilization Management Plan.
  • Develop, review, and revise the Utilization Management Plan on a yearly basis.
  • Engage with payors and vendors in the management and tracking of denials and appeals, as appropriate.
  • Maintains a reduced case manager case load.
  • Oversees and participates in departmental budget process.
  • Supervises Case Management Staff in daily functions.
  • Oversees and participates in the hiring and supervision of departmental staff and conducts staff performance evaluations.
  • Directs the hospital's Utilization Review and Case Management functions.
  • Participates in the design and evaluation of information systems that facilitate data acquisition and management.
  • Participates in the development and implementation of the hospital's educational objectives as it relates to Case Management and Utilization Review.
  • Assesses the potential impact of regulatory changes as they relate to Case Management and Utilization Management and advises Administration and Department Managers accordingly.
  • Develops and reviews departmental policies and procedures to reflect the Hospital mission and philosophy.
  • Liaison to clinical departments identifying and assessing problems and monitoring actions.

Qualifications

  • Licensed & Registered RN in the State of New York
  • Minimum of a Bachelor's Degree in Nursing. Master's Degree preferred.
  • Case Management certification required within 2 years of employment.
  • Minimum of five (5) years experience in Nursing and minimum of five (5) years of experience in Case Management, Utilization Review or Discharge Planning required. Two (2) of these years must be in a supervisory position.
  • Knowledge of and experience with hospital and patient information systems
  • Ability to work collaboratively and effectively with individuals from different disciplines in varying positions within and outside the organization.
  • Ability to develop management systems to track data over time.
  • Ability to design and conduct clinical research activities and perform data analysis.
  • Excellent written and verbal communications skills
