Description: POSITION SUMMARY:The RN/DON is responsible to the Director of Administrative Services, and Home of Hope, Inc. clients for rendering nursing care in assigned area in terms of individualized client needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of nursing.POSITION RESPONSIBILITIES / STANDARDS:Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned client/family as evidenced by documentation and observation of positive client care outcomes.Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards as observed by the registered nurse.Assist in data collection on the Admission Data Record.Contribute to the development of the client/significant Individualized Habilitation Plans (IHP) based on the identified client problems/needs.Carry out physician orders accurately, directly and indirectly.Assist in the evaluation of the effectiveness of the nursing care administered relative to the client/ significant other identified problem/needs.Document pertinent observation and intervention on appropriate record.Perform selected tasks related to client care and the client's environment in a safe manner.May be required to administer medication accurately, observing client response, as evidenced by documentation in the medical record and lack of negative outcomes.Maintain and coordinate communication among client/significant other, staff, and physician as evidenced within medical record, client, and health care team feedback.Assist in assuring that effective communication exists with client/significant other as reflected in the IHP.Review change of shift reports consisting of pertinent and relevant information for assigned clients.Effectively communicates clients changing needs and IHP among staff members.Report client needs/problems and observations regarding client's condition to the physician.Communicate as needed with client/significant other information regarding IHP and therapeutic plans.Work collaboratively with other members of the health care team in coordination of the client's care as evidenced by other department feedback.Provide client/significant other health teaching as delegated by and observed by the RN.Utilize appropriate teaching methods such as one-on-one and group instruction, demonstration, or discussionDocument health teaching including client/significant other response to and their understanding of health teaching.Participate in staff development programs as evidenced by Team Member education record.Attend scheduled division/unit staff meetings and is responsible for information discussed.Responsible for ensuring staff has received training to do their jobs sufficiently, i.e. lifting, non-violent, clinical skills, etc.DIRECTOR OF NURSING DUTIES:May have access to all the facility's medication supplies.Must approve someone to return to work who shows signs and symptoms of a communicable disease.Review all incidents where health and safety of the clients are affected.Will be assigned to the Quality Assurance Committee.Must be available to destroy medications with the pharmacist.Assist in developing policies and procedures for the administration, storage and destruction of medications with the pharmacist and administrators.Act as the infection preventionist.Supervises all nurses.CLIENT SATISFACTION:Assist clients, family members or other clients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.Answer and refer telephone calls or other inquires to ensure accurate and timely communications are facilitated.TEAMWORK:Assist in the orientation/training of new staff members.Consistently work in a positive and cooperative manner with fellow Team Members.Assist other Team Members in the performance of their assignments.Recognize the need for variations in staffing and volunteer to fill open shifts.Take direction and initiate actions (cross/additional training) that will allow the assumption of cross-functional duties to ensure seamless client care.Demonstrate flexibility to perform duties wherever volume deems it necessary within the organizationPROBLEM SOLVING:Demonstrate sound judgment by taking appropriate actions regarding questionable findings or concerns.Investigate and follow through on unusual orders or requests for service or information.Follow proper reporting procedures for actual or potential accidents and/or incidents so follow-up and/or prevention can occur.Record/report the need for service maintenance or repair of equipment and remove any faulty equipment from service.Consistently evaluate work and determine if further steps are needed to meet client expectations.Take initiative to do or redo inadequate or incomplete work, even if it is not yours.Ensure compliance with regulatory standards.PRODUCTIVITY/EFFICIENCY:Consistently demonstrate ability to respond to changing situations in a flexible manner in order to meet current needs, such as reprioritizing work as necessary.Minimize non-productive time and fill slow periods with activities that will enable you to prepare to meet the future needs of the System (education, organizing, and housekeeping, assisting others).Organize job functions and work area to be able to effectively complete varied assignments within established time frames.Consistently demonstrate ability to take the initiative to make decisions/choices without direct supervision.PERSONAL DEVELOPMENT:Attend required annual in-service programs.Participate in continuing education activities and obtain required contact hours.ADHERENCE TO DEPARTMENTAL POLICIES:Demonstrate knowledge and understanding of all policies and procedures and ability to reference them from appropriate books and manuals.Comply with the Home of Hope's infection control policies and procedures including Body Substance Isolation (BSI) and the Exposure Control Plan which reflects OSHA Blood borne Pathogens Standard to ensure a safe working environment for self and others.EDUCATION:Graduate of an accredited school of Nursing.Valid registration as a RN in the State of Oklahoma.Current AHA Healthcare Provider training (BCLS).EXPERIENCE:One-year experience in geriatric nursing and/or area of specialty preferred.PHYSICAL DEMANDS/WORKING CONDITIONS:Intermittent physical activity including walking, standing, sitting, lifting and supporting clients.Incumbent will be exposed to virus, disease and infection from clients and specimens in working environment.Incumbent will be required to work at any facility and be responsible for own transportation.Incumbent will be scheduled based on operational need. (Rotate shifts, standby, and on-call)Incumbent may experience traumatic situations including psychiatric, diminished, aging and deceased clients.DON shall be on duty on the day shift and be responsible for all resident care including, but not limited to, the physical, mental and psycho-social needs. When necessary, the DON may work other than the day shift but no more than three shifts per week and shall not exceed three consecutive weeks in a three-month period.Shall be available by telephone when needed by the facility.Requirements:PIf9feb2c717d5-26289-#######0
