Join our team of dedicated professionals who are committed to saving lives.Summary:Primary responsibility is to utilize the resources available and coordinate information with appropriate hospital and LifeLink staff to evaluate and respond to hospital referrals for the purpose of determining a potential donor. Apply LifeLink's policies and procedures in performing and documenting the approach, authorization, honoring donor designation and medical/social behavioral interview of next-of-kin and other appropriate family members/associates to offer the option of donation for the purpose of transplantation, medical therapy or research. Participate in call schedule rotation to ensure coverage of this position's responsibilities 24/7. Assist supervisor or management as directed in improving processes, training, and other duties as assigned/directed including taking referral call, starting cases, import offers and Puerto Rico cases. Responsibilities:
Participate primarily in the Vascular FCC call schedule coordinating FCC responsibilities with other vascular coordinator staff.Consistently maintain or exceed the team goal of authorization rate.Referral, pre-authorization and authorization responsibilities will consist of but may not be limited to:- Referral evaluation of potential DCD or BDD,
- Determination of suitability, in concert with the AOC and MDOC,
- Verification of name and location of next-of-kin, disclosing donor designation
- Effective communication with the AOC and on call team with all potential donor situations
- Completion of physical assessment utilizing approved forms,
- Review of patient's hospital chart,
- Review of donor information with appropriate hospital personnel,
- In conjunction with AOC determine appropriate time for discussions with family and requesting authorization from NOK or disclosure of donor designation utilizing approved authorization and medical/behavioral history forms and protocols.
- Timely weblog entries of referrals and other details related to case related family dynamics
4. Post-authorization responsibilities will consist of but may not be limited to:
- Offering family all appropriate information,
- Notifying the call team of authorization request outcome,
- Provide follow-up with family in conjunction with vascular coordinator, and administrator on call.
- Provide follow-up to appropriate Hospital Development Liaison.
- Review of and co-sign family letters when appropriate to do so.
5. Case initiation responsibilities will consist of but may not be limited to:
- Initiating a donor chart,
- Write beginning orders, obtain blood and nodes for infectious disease testing and HLA tissue typing and proceeding forward with entering data in UNOS,
6. Assist with mentoring of new staff.7. Assist with special projects.8. Assist with QAPI.9. Monitor program updates to ensure all policy and operational manuals remain current and that all appropriate external communication has occurred.10. Participate actively in hospital development initiatives such as AAR and DAC's.11. Participate in continuing education courses and seminars necessary to obtain appropriate CEUs to maintain certifications and licensure.12. Participate in regularly weekly OPO meeting, CCR's, FCC meeting, QA meetings or others as directed.Requirements:
Minimum of two years in a related LifeLink position and/or appropriate credentialing and experience which may include medical-related degree/certification or appropriate experience in a medical environment or social services field.Demonstrated above average interactive verbal communication skills.Availability to handle rotation of 24/7 call responsibilities and a work schedule that may require response to emergency back-up call coverage.Automobile and/or air travel on a regular basis.Current driver license with good verifiable driving record.Understanding that confidentiality must be maintained, according to Foundation protocols.[Bi-lingual abilities a plus]Working Conditions:Local/State auto/air travel is required. Variable/rotation on-call days/hours including nights/weekends/holidays depending on call schedule and case duration. Possible high stress / extended hours while on call.
