General Description:

The primary responsibility of the Field Registered Nurse is for the provision of directed skilled nursing care for the coordination with other members of the health care team and direct supervision of the agency's provided paraprofessional services. This position provides in-field professional supervision and on the job training to home health aides. The field Registered nurse also assists in the agency's continuing education/in-service program & field competency testing of all clinical staff.

Qualifications:

NYS licensed & currently registered as a Registered Professional Nurse

One year experience in the provision of nursing care in an acute care facility.

One year satisfactory nursing experience in a home care agency, desired.

· Ability to perform mathematical operations as related to medication calculations, units of American money and weight measurement, volume and distance.

· Good organization writing, communication and interpersonal skills.

· Ability to solve practical problems & deal with a variety of concrete variables in situations where only limited standardization exist. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.

Essential Duties/Responsibilities:

· Conducts assessments & reassessments of patient's receiving services i.e., OASIS

· Develops service plans, plans of care & collaborates with the Case Manager RN as appropriate.

· Provides patients/families with information regarding agency services, fees, Bill of Rights, Advance Directives & agency policies.

· Coordinates al care & services provided to the patient.

· Communicates findings with the patient's physician & obtains verbal orders as necessary. Develops & implements treatment plans in conjunction with the patient's authorized medical practitioner.

· Consults with DPS to maintain the highest quality care.

· Initiates written Medical Orders & written reports to the patient's physician as necessary.

· As necessary conducts home visits to provide intermittent skilled nursing services in accordance as ordered by Physician.

· Monitors the patient's medication regimen for drug-drug/drug-food interactions, side effects, & adverse reactions.

· Performs patient re-assessments in the home. Reassesses patient needs for services on a continual basis & discusses any changes in the plan of treatment with patient physician/family.

· Conferences with all involved disciplines in the plan of care. Conducts on-site competency review as part of supervision activities.

· Prepares written documentation & summaries of the patient's status & progress as needed for continuation of services. Document accurately & completely the patient's clinical record. Conferences with all clinical & scheduling staff to assure continuity of patient care. May utilize electronic means.

· Plans, facilitates, & documents patient discharge from agency.

· Becomes familiar with all resources necessary to provide a complete coordinated care program for the patient, e.g respiratory, DME, etc.

· Initiates referrals for other services or community resources as needed.

· Maintains updated professional knowledge & participates in patient education programs to ensure optimum quality of care. Participates in the agency's in-service program. Provides in-service education to professional/paraprofessional staff.

· Participates in the agency's Quality Management Program.

· Assists training program in classroom observation & competency testing of trainees.

· Reviews & responds to complaints, grievances & incidents under direction of DPS.

· With other nurses, provides 24-hour on call coverage on a rotation basis as needed.

· Possess customer service/interpersonal Skills

· Possess specialized skills & technical competencies

· May perform other duties as assigned by DPS.

Contact:

Most frequent contact will be with patients, patient families, vendors, clinical staff and HHA (home health aides).

Nature or Purpose:

Care issues, provide direction, coordination & supervision, address patient and program issues.

Work Environment:

Office, patient homes in Brooklyn or Queens borough area

Salary:

$95K-105K
