Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care;Communication. Counsel and guide individuals and families towards self-help in recognition and solution of physical, emotional and environmental problemsEvaluation & Documentation. Conduct reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to all home care staffEDUCATION:AS: Nursing (Required)PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.LICENSES/CERTIFICATIONS:RN - Registered Nurse - New York State Education Department (NYSED)BLS - Basic Life Support - American Heart Association (AHA)PAY RANGE: $73,528.00 - $98,072.00The listed base pay range is a good faith representation of current potential base pay for successful applicants.
