MGB-13493- RN Home Health - Newton, MA - 13 Weeks Contract Sigma Healthcare team is currently looking for a RN Home Health to work onsite at our client's healthcare facility in Newton, MA. Shift: 08:00 AM- 04:30 PM Summary:
- As a Home Health RN, responsibilities not limited to assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families.
- Patient care is delivered within organizational philosophy, policy and standards of community health practice.
- Also required to perform admission processes and case management responsibilities in accordance with MGB Home Care's policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.
Requirements:
- 1 year Traveler and Home Care experience.
- 1 year Case Management Experience.
- Ability to travel to cases as assigned.
- Currently BLS certified
- Current valid state RN licensure.
