  • Job Summary
    • Exercise intelligent leadership in the provision of nursing service necessary to support, maintain, and implement departmental objectives under the direction and supervision of the Quality/Compliance Supervisor or Director of Home Health/Hospice.. Provides, arranges for, teaches or supervises nursing care of the home health patient. Carries out nursing skills contributing to the treatment and rehabilitation. Appraises individual and family health needs and hazards - existing or potential. Plans coordinated Home Health service for assigned patients. Carries out treatment, diagnostic and/or preventive procedures, as ordered by the patient's physician.
  • Job Specifications
    • EDUCATION AND EXPERIENCE: Currently licensed RN in the state of Illinois by the Illinois Department of Professional Regulation. A minimum of one year's experience is preferred. Must have a valid drivers license if applicable.
    • JOB KNOWLEDGE:
      • Comprehensive knowledge and skill in current nursing practice.
      • General knowledge of current community health principles.
      • Knowledge of the role of the professional nurse as a team leader and contributing constructive leadership.
      • The patient populations must be considered when assessing job knowledge required from the employee.
    • ABILITIES, INTERESTS AND TEMPERAMENTS:
      • Ability to organize, manage, and to provide skilled nursing care based on scientific principles, basic behavioral concepts, and intelligent professional judgment.
      • Ability to recognize the need for adjustment and flexibility in nursing care functions so as to maintain quality nursing care in all types of situations.
      • Ability to exercise leadership coupled with tolerance and understanding as well as with appreciation and respect for the special abilities and skills of others.
      • Ability to accept and constructively utilize supervision.
      • Ability to establish and maintain positive working relationships with individuals and families.
      • A commitment to strive for an improvement of nursing skills and greater competencies in the Home Health field.
      • Capable of developing and maintaining positive working relationships with colleagues, physicians, patients, families, and others, enlisting support and cooperation from same.
      • Demonstrates commitment and exemplary loyalty to the hospital and the Home Health Care department.
      • Must have the ability to work with a variety of patients of all populations (e.g. developmentally appropriate care, cultural diversity, patients with pain, victims of abuse or neglect, and diagnosis or procedure specific such as patient with congestive heart failure or cesarean delivery).
    • The RN will successfully complete orientation and hospital-wide and department-specific CBL's (computerized based learning). The RN will participate in departmental inservices and will demonstrate competency by completing the service specific competency testing / evaluation process, as appropriate to the RN role. Continuing education programs will be offered to augment the RN's knowledge of pertinent developments in patient care and to foster a performance improvement environment.
