The Home Health Registered Nurse is responsible forproviding and documenting skilled one-on-one nursing care in accordance withthe physician-ordered individualized patient care plan. This care is provided mostly in the patient'shome or a residential setting but can transfer outside the home duringappointments and other activities the nurse may accompany the patient to.
Essential Duties and Responsibilities: - Completes an initial assessment of patient and family todetermine home care needs when applicable
- Provides a complete physical assessment and history ofcurrent and previous illness(s)
- Regularly re-evaluates patient nursing needs
- Initiates the plan of care when directed and makes necessaryrevisions as patient status changes
- Uses health assessment data to determine nursing diagnosis
- Establishes goals based on nursing diagnosis andincorporates therapeutic, preventative, and rehabilitative nursing actions
- Includes the patient and the family in the planning andexecution of the treatment plan and goals
- Initiates appropriate preventative and rehabilitativenursing procedures
- Administers medications and treatments as prescribed by thephysician
- Provides healthcare instructions and education to thepatient as appropriate per assessment and plan of care
- Identifies discharge planning needs when applicable
- Adheres to home health agency policy and protocol
- Attends case conference, team meetings, and educationin-services as required
Minimum Requirements: - Graduate of an accredited school of nursing
- Currently licensed as a Registered Nurse in the state inwhich the RN will practice
- Complies with all relevant professional standards ofpractice
- Participation and completion of Amergis' Competency programwhen applicable
- Current CPR if applicable
- TB questionnaire, PPD or chest x-ray if applicable
- Current Health certificate (per contract or stateregulation)
- Must meet all federal, state and local requirements
- Successful completion of new hire training as applicable tojob site
- Understand patient confidentiality and HIPAArequirements
- Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
- Computer proficiency required
- Must be at least 18 years of age
