Salary Range: $29.70-41.85
Position Summary: Provide nursing careand coordination of services, and health teaching to patients and families intheir place of residence. Identifies problems that may adversely affect patientoutcomes and uses knowledge, critical thinking skills, and equipment to troubleshoot and resolve them. Provides supervision of Home Care Aides in the homeenvironment. Provides community health services and educational programs to thegeneral public as indicated. Carries out supervisory responsibilities inaccordance with the organization's policies and applicable laws.Responsibilities include orienting and training competent employees andappraising performances.
Minimum knowledge,skills, and abilities: - Qualifications: To perform this job successfully, an individual must have their own cell phone and be able to perform each essential function satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required
- Work experience: Bachelor's Degree (B.S.) from four-year college or university, or two years of work related experience and/or training, or equivalent combination of education and experience
- Education/Certifications/Licenses: Valid Iowa Nursing License, Valid Iowa driver's license, maintains auto liability insurance. BCLS certification
- Desired Work Traits: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables. Ability to calculate discounts, interest, proportions, percentages, area circumference and volume. Ability to apply concepts of basic algebra and geometry
- Communication Skills: Ability to effectively present information and respond to questions from groups, patients, families, customers, and the general public
Essential Functions: - Regular Attendance
- Demonstrate knowledge of critical thinking skills necessary to provide care appropriate to the ages of the patients served within the department
- Demonstrate good time management and organizational skills
- Perform patient assessments including but not limited to temperature, pulse, blood pressure and other vital signs. Record significant changes in patients' condition and notify physician of patients' condition and progress or lack of progress toward goals of care. Maintains awareness of comfort and safety needs of patients. Administers and or supervises prescribed medications and treatments. Explains procedures and treatments to patient and family and provides teaching and education as needed. Develops a Plan of Care based on the individualized needs of the patient and family. Coordinates care of patients with other providers to achieve patient centered goals
- Responds to life saving situations based upon nursing standards, policies, procedures, and protocol
- Obtains medical history and performs and documents complete assessment using required assessment tool, including a medication regimen review on each patient assigned
- Leads and/or participates in quality improvement activities of the department
- Maintain performance in compliance with State and Federal laws, and consistent with high standards of business and professional ethics
- Provides clinic and health promotion activities
- Participates in department staff meetings and hospital wide in-services as required. Participate in department continuous quality improvement efforts. Maintain professional growth and development. Maintain a positive attitude and professional manner
- Maintain effective communication with staff, medical staff, and all levels of supervision
- Demonstrate ability to perform basic computer skills
- Must be able to be on call as needed
- Provide public health duties as indicated
- Demonstrate ability to accurately document in the Electronic Medical Record
WORKING CONDITIONS: Hours may vary depending on patient census. The employee is regularly exposed to outside weather conditions including driving. The employee is also exposed to varied home environments including pets and household pests, and cluttered conditions. The employee is occasionally exposed to wet and or humid conditions, moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electric shock. The noise level in this work environment is usually moderate, but can vary with each home situation.
