About Palliative
In collaboration with the entire interdisciplinary team, assess, plan, implement, coordinate, monitor and evaluate the strategies and services required to meet a patient/family's needs. An outcome-based plan of care is created and continually adjusted through utilization of the nursing process, relationship centered care principles and holistic philosophy assuring the changing needs of the patient/family are met.
Scope of work
Under the direct supervision of the Nursing Manager, and in collaboration with the interdisciplinary team, the Nurse uses clinical judgment skills to access and implement the treatment plan for patients. The RN assesses monitors, detects, plans and takes appropriate action to prevent physiological complications associated with specific health problems and/or medical treatment.
