Work 32hrs / Paid 40hrs

Friday, Saturday, Sunday & Monday

This position is accountable for ongoing planning, organization, direction, evaluation and safe delivery of high-quality care to adult, geriatric, pediatric, and neonatal clients in accordance with Policies and Procedures, State and Federal Regulations.

Essential Duties and Responsibilities:

  • Accepts appropriate referrals to the agency. Assigns qualified professional personnel to meet the needs of the adult, geriatric, pediatric, and neonatal client referred. In conjunction with clinical staff, prepares on-call schedules, plans and organizes daily assignments including the review and assignment of new patient referrals.
  • Supervises all assigned hospice and palliative nursing personnel in the delivery of nursing services to clients and families. Assigns work to subordinate staff, provides direction and reviews work. Participates in and provides coordination and management of all professional services rendered to clients and families by agency staff.
  • Supervises the maintenance of a balanced caseload and organizes the flow of work, utilizing clinical staff and makes field assessments as needed. Performs periodic home visits with staff for the purpose of role modeling, problem solving, supervising and evaluating of performance/quality of care.
  • Assumes an active role in the professional development of assigned staff, including participation in orientation, ongoing skill development and other mentoring activities. Responsible for maintaining an office environment that fosters the professional growth and development of assigned staff.
  • In conjunction with Human Resources, participates in staff recruitment, selection and discipline/termination of employment of staff not performing according to agency standards (requires consultation with administration).
  • Conducts clinical record reviews to assess the appropriateness of care plan, the utilization of services and to ensure complete documentation of needed services. Participates in program planning and evaluation activities related to the clinical services of the agency.
  • Demonstrates timely and accurate computer entry of patient/job information to facilitate the development of the OASIS data set, hospice item sets, orders for service, creation of time sheets and other required patient/employment documents and reports.
  • Participates in Administrative Call on a rotating basis with other members of the management team.
  • Participates in program planning and evaluation activities related to the clinical services of the agency.
  • Communicates effectively and tactfully with clients, recognizing their age, cultural diversity, needs, abilities and physical condition.
  • Can be depended upon to report to work on-time, use time off appropriately, and complete annual education & medical requirements.
  • Performs related and other duties as required.
  • Minimum Qualifications:

    Education: A Baccalaureate Degree from an NLN accredited School of Nursing and a minimum of two (2) years of full-time clinical experience within the past five (5) years in a home care agency or related health care facility which included care of the sick; OR

    A Diploma in nursing or an associate's degree in nursing and

    A. A minimum of three years of full time or full time equivalent clinical experience in nursing within the past five years, at least one of which was in a home health agency or community health program which included care of the sick at home; and

    B. Evidence of certification by the American Nurses' Association as a community health nurse or completion of at least six credits received within two years in community health nursing theory or six credits in health care management from an accredited college or university program or school of nursing.

    Experience: Knowledge of State Regulations, Medicare and Medicaid standards as related to Hospice services.

    Certificates, Licenses, Registrations: Current Connecticut Registered Nurse License. Maintains current Drivers' License and auto insurance.

    Other: Possess reliable transportation (Exception: Employee who utilize public transportation—Supervisory approval needed).
