The Registered Nurse provides and coordinates patient care, educate patients and the public about various health conditions, and provides advice and emotional support to patients and their families.
Job Summary:
The Registered Nurse Works collaboratively with providers and other health care staff with responsibility for clinical activities involving routine and complex nursing care and patient flow, patient recordkeeping and follows-up, patient education, and other related activities as required.

(As mentioned in the Title 24 -1900 Board of Nursing) The Registered Nurse shall conduct and document nursing assessments in accordance with the nursing process. The registered nurse shall assess the health status of individuals and groups by: Collecting objective and subjective data from observations, examinations, interviews, in person or by electronic communication and written records in an accurate and timely manner. The data include but are not limited to: Biophysical and emotional status and observed changes; Growth and development; Ethno-cultural, spiritual, socio-economic and ecological background; family health history; Information collected by other health team members; Ability to perform activities of daily living. Consideration of client's health goals. Client knowledge and perception about health status and potential, or maintaining health status. Available and accessible human and material resources; Patterns of coping and interaction. Sorting, selecting, reporting, and recording the data. Analyzes data. Validating, refining and modifying the data by using available resources including interactions with the client, family, significant others, and health team members. Registered Nurses shall establish and document nursing diagnoses that serve as the basis for the strategy of care. Registered Nurses shall develop strategies of care based on the nursing process. This includes: Prescribing nursing interventions. Initiating nursing interventions through giving, assisting and/or delegating care. Identifying priorities in the plan of care. Setting realistic and measurable goals for implementation. Identifying measures to maintain comfort, to support human functions and responses, to maintain an environment conducive to well-being, and to provide health teaching and counseling. Registered Nurses shall participate in the implementation of the strategy of care by: Providing care for clients whose conditions are stabilized or predictable. Providing care for clients whose conditions are critical and/or fluctuating, under the direction and supervision of a recognized licensed authority. Providing an environment conducive to safety and health. Documenting nursing interventions and client outcomes. Communicating nursing interventions and client outcomes to appropriate health team members. Registered Nurses shall evaluate outcomes, which may include the client, family, significant others and health team members. Evaluation data shall be documented and communicated appropriately. Evaluation data shall be used as a basis for modifying health care strategies including but not limited to reassessing client health status, modifying nursing diagnoses, or prescribing changes in nursing interventions.
Requirements:
Education and/or Experience:
* RN from an accredited school of nursing; BSN preferred.
* Minimum of one year of professional nursing experience, clinic or physician's office preferred.
* Delaware R.N. License
* CPR/BLS certification required
Language Skills:
Bilingual skills preferred (English/Spanish).

Knowledge of established nursing concepts, principles and practices. Knowledge of wellness, illness, growth and development, human behavior and psychosocial factors. Knowledge of computer systems and standard Outlook and office applications. Knowledge of medical record documentation requirements, EMR, and federal/state laws regarding release of medical records or health care information.
Skill in time management, problem solving, multitasking, prioritizing and medical care coordination. Skill in initiating appropriate crisis interventions and emergency response. Skills in patient triage in person and on the phone. Skill in dealing with large quantities of information in an organized manner. Skill in using computer and medical records software. Skill in posting all pertinent information, laboratory and test results information into EMR, meeting Center standards for timeliness. Skill in medical procedures and practices.
Ability to determine and assess the needs of the patient. Ability to engage patients and families in the development and implementation of a care plan. Ability to read and interpret physician orders and notes from other providers. Ability to function effectively within a team. Ability to use excellent interpersonal skills in building rapport, trust, and patient . centered care. Ability to handle large volume of detail-oriented work in a stressful environment. Ability to analyze EMR for completeness and accuracy, paying attention to detail. Ability to manage time and interact with patients, staff and the public in a professional manner.
Equipment Operated:
Wide range of standard medical equipment including portable Ultrasound equipment, syringes, speculum, oxygen tanks, wheelchairs, thermometers, etc. Office equipment including computers, fax, copy machines and shredders. Computer use and proficiency required.
Working Conditions:
Medical exam/treatment rooms and medical offices in Health Center. Controlled lighting, air quality, and temperature settings. Environmental hazards may be unpredictable including exposure to communicable diseases and biohazards.
Physical/Mental Requirements:
Involves standing, walking, kneeling, bending, grasping, manipulating and squatting and lifting (up to 50 pounds). Walking or standing may be more than four (4) hours per day. Must be able to work in a standard medical office setting including use of a computer, and standard office equipment. Must have ability to read computer screens, printed materials and hearing and speech to communicate in person as well as over the phone. Must be able to attend meetings at various sites when required. Stress can be triggered by workload. Work is at times sedentary.
