Provide direct and indirect nursing care to assigned patients. Works under the direct and indirect supervision of the Clinical Services Coordinator and/or Medical Director but is independent in the application of advanced nursing knowledge and skills. Manages complex clinical situations for which he/she is responsible. JOB RESPONSIBILITIES:
- Attends morning staff meetings and updates clinic schedule as needed.
- Conducts initial, periodic and unscheduled nursing assessments of participants.
- Attends Interdisciplinary Team (IDT) meetings and shares information about patients' condition, problems, needs and priorities.
- Participates in development of plans of care that integrate information from all disciplines.
- Documents changes in participants' status and communicates changes to IDT members.
- Reviews documentation of home care worker and home care supervisor visits and reports through the clinical and IDT as needed.
- Provides nursing care according to accepted practice standards.
- Educates participants, families and care givers about the management of medical conditions.
- Identifies emergency situations and initiates therapy as appropriate.
- Respects participants' rights and treats patients in accordance with ethical standards.
- Completes all documentation in the medical record and/or electronic health record (EHR) as required.
- Acts only within the scope of his/her authority to practice.
- Other job related duties as assigned.
