Job Type Full-timeDescriptionCHJC is looking for a RN to join the team. The agency has a robust benefits package and a very desirable schedule. This is a salaried position for DAYS only. GENERAL STATEMENT OF DUTIES: Responsible for rendering nursing care to the service recipients of Children's Home of Jefferson County (CHJC) in terms of individualized client needs. The role includes coordinating care for foster youth, working within an interdisciplinary team across the CHJC care continuum, supporting the Therapeutic Crisis Residence Program (TCRP), and abiding by all OMH (Office of Mental Health) and OCFS (Office of Children and Family Services) regulations.ESSENTIAL FUNCTIONS OF THE JOB: The major responsibilities of this position include, but are not limited to, the following:
  • Conducting health assessments, foster care treatment plans for foster youth, within the Mental Health clinic, or within TCRP.
  • Completing chart documentation and progress notes to maintain accurate and up-to-date records.
  • Reviewing and auditing medication logs regularly within TCRP to ensure accuracy and compliance.
  • Medication management: Review medication books in all necessary cottages (TCRP), ensure medication pill packs are up to date, check pill counts. Sign off on the MAR confirming completion and promptly email significant findings to the appropriate Program Director.
  • Transporting residents to and from medical appointments and completing necessary paperwork and progress notes.
  • Coordinating and transporting residents to initial oral surgical appointments and specialized out-of-county medical appointments.
  • Facilitating eyeglass repairs for residents by picking up and delivering them for repairs as needed.
  • Scheduling medical appointments
  • Attending Team Meetings to collaborate with the interdisciplinary team.
  • Developing, reviewing and implementing nursing policies and procedures to ensure quality care and compliance with regulations.
  • Participating in agency-provided and external training as requested or required to maintain professional competence.
  • Staying informed of changes in the nursing profession to ensure current practices align with professional standards.
  • Entering medical data for all residents into the State's Connections System in a timely and accurate manner.
  • Collaborating across CHJC's care continuum to ensure coordinated and comprehensive care for foster youth.
  • Supporting the TCRP program to provide targeted and effective care.
  • Abiding by OMH and OCFS regulations to ensure compliance with all state requirements and standards.
  • QUALIFICATIONS:
    • Licensed Registered Nurse (RN) in New York State.
    • Experience in pediatric or adolescent care preferred.
    • Strong organizational, communication, and teamwork skills.
    • Ability to work effectively within an interdisciplinary team.
    • Commitment to fostering a safe and supportive environment for residents.
    Physical Demands: Standing, walking, memory, concentration, sitting for long periods of time, hearing, carrying, speaking, writing, bending, pulling, problem solving, lifting, reaching, kneeling, running, seeing, reading, climbing, pushing. Works in a regular office environment, exposure to noise, blood and/or body fluids.Position Type/Expected Hours of Work:Day Shift OnlyTravel: Travel may be required.Other: Must be able to provide crisis intervention. Must be able to lift 50 lbs. to waist high. Must complete the Onboarding Training Program sponsored by the Agency. Successful completion of routine pre-employment requirements as mandated by the New York State Department of Health, Office of Mental Health and Office of Children and Family Services along with ongoing clearances and exclusion checks. These requirements are to include employment health requirements, Covid-19 vaccination or exemption, reference checks, criminal background checks/fingerprinting, State Central Registry clearance and your eligibility to work in the United States.
