**Available to regular status employees who are scheduled to work a minimum of 24 hours. Southcoast Health is an Equal Opportunity Employer. Compensation: Pay rate will be determined based on level of experience. Responsibilities Under direction of the VP of Post-Acute Care or designee, perform diversified duties including but not limited to communicating with Skilled Nursing Facilities for bed offers for Southcoast Hospitals. Communication with case mangers in SLH, CMH and Tobey bed offers from SNFs for the placement of patients from the acute care to SNFs. Manage the referral to bed offer for the entire process. Manage information flow regarding patient needs and issues related to patient discharge process, scheduling, insurance issues etc. Create a strong relationship and rapport with the SNF accounts to gain as many beds offers as possible to move patients out of the acute care environment as quickly as needed. Document bed offers, bed flows, discharge calendars in each SNF for the purpose of creating awareness around access to care. Qualifications
  • Current licensure by Board of Registration in Nursing (LPN or RN) is required.
  • One to three years nursing experience is required.
  • At least two years of experience in an SNF/Sub acute care facility is required.
  • Preferable in an administrative / admissions role Must possess and demonstrate highly effective interpersonal, customer service and communication skills.
  • Requires proficient computer skills, including experience with various software, database, and electronic records programs.
  • Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
Pay Range USD $40.74 - USD $72.46 /Hr. Rotation Schedule Requirement some weekend and holiday rotation as needed
