They are responsible for managing the overall health and well-being of patients, ensuring personalized care plans, and collaborating with interdisciplinary teams to promote optimal outcomes.Key Responsibilities:
  • Provide direct nursing care to residents in accordance with individualized care plans.
  • Administer medications, perform treatments, and monitor patients' progress, ensuring safety and adherence to established protocols.
  • Assess, plan, implement, and evaluate nursing care for long-term care residents.
  • Collaborate with physicians, therapists, and other healthcare professionals to develop and adjust patient care plans.
  • Monitor and document residents' health status, including vital signs, medication administration, and overall progress.
  • Educate residents and their families on health conditions, care needs, and treatment options.
  • Respond to and manage acute and chronic conditions, ensuring timely interventions and care coordination.
  • Provide emotional support and counseling to residents and their families, promoting a positive living environment.
  • Ensure compliance with all federal, state, and facility regulations related to long-term care.
  • Supervise and delegate tasks to licensed practical nurses (LPNs) and nursing assistants (CNAs).
  • Participate in quality improvement initiatives, staff training, and continuing education.
  • Maintain accurate patient records and documentation, adhering to established clinical standards and protocols.
  • Address concerns related to infection control, safety, and preventative care to minimize risks to residents.
Qualifications:
  • Education: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.
  • Licensure:
    • Valid Registered Nurse (RN) license in the state of practice.
  • Certification:
    • Basic Life Support (BLS) certification required.
    • Advanced certifications such as ACLS or Geriatric Nursing (e.g., CGRN) preferred.
  • Experience: Previous experience in long-term care, gerontology, or related setting
