PureHealth Transitional Care at THR Arlington -

Full time day shift: 6:00 am-6:00 pm

The RN/LVN is responsible to the Unit Manager and its clients for rendering nursing care in assigned area in terms of individualized customer needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the Registered Nurse. Shifts are 6am-6pm or 6pm-6am.

Essential Duties and Responsibilities1. Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned customer/family as evidenced by documentation and observation of positive customer care outcomes

2. Initiates and leads team conferences in development of individualized nursing care plans

3. Assists with or institutes emergency measures for sudden adverse developments in customers

4. Contribute to the development of the customer/significant other plan of care based on the identified issues, directly and indirectly

6. Assist in the evaluation of the effectiveness of the nursing care administered relative to the customer/ significant other identified problem/needs

7. Document pertinent observation and intervention as appropriate in the EMR

8. Perform selected tasks related to customer care and the customer's environment in a safe manner

9. Administer medication accurately, observing customer response, as evidenced by documentation in the EMR and lack of negative outcomes

10. Perform I.V. Therapy procedures on those customers receiving peripherally inserted Intravenous fluids, pursuant to RN scope of practice.

11. Maintain and coordinate communication among customer/significant other, team members, and physician as evidenced within the EMR, client, and health care team feedback.

  • Assist in assuring that effective communication exists with customer/significant other as reflected in the plan of care
  • Give punctual change of shift report consisting of pertinent and relevant information for assigned customers
  • Effectively communicate customers changing needs and plan of care among team members
  • Report customer needs/problems and observations regarding customers condition to Unit Manager, ARNP, and Physician
  • Reinforce customer/significant other information regarding plan of care and discharge plans
  • Work collaboratively with other members of the health care team in coordination of the customer's care as evidenced by other department feedback

12. Provide customer/significant other health teaching as indicated by condition and need

13. Use available written and AV materials specific to customer condition or disease

14. Utilize appropriate teaching methods such as one-on-one and group instruction, demonstration, or discussion

15. Document health teaching including customer/significant other response to and their understanding of health teaching

16. Participate in staff development programs as evidenced by education record

17. Demonstrate knowledge and understanding of all policies and procedures and ability to reference them from appropriate resources

18. Comply with the infection control policies and procedures including ensuring a safe working environment for self and others

19. Attends and participates in in-service training.

Work EnvironmentThis job operates in a post-acute setting. This role requires regular walking to various locations around the facility. This role also routinely comes into contact with patients who may have contagious illnesses. Team member is expected to adhere to facilitys no-smoking policy.

Physical DemandsWhile performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling, and stooping all day. The employee must frequently lift and/or move items over 60 pounds.

Position Type and Expected Hours of WorkThis is a position in a post-acute setting that is open 24 hours a day, 365 days a year. Weekend and holiday shifts will be required.

3. Proficient computer and Microsoft Application skills. Must have an ability to learn new systems

4. Ability to work with a culturally diverse population

5. Must have positive communication skills and demonstrated adequate maturity and patience

6. Proficient in the English language. Excellent communication (verbal and written), customer service and interpersonal skills

7. Must have good organizational skills with attention to detail

8. Must be able to successfully prioritize workload, demonstrate initiative, and analyze situations to make sound decisions

9. Must be observant and recognize cleaning needs

10. Must show initiative and take action on observed needs

11. Self-directing with the ability to work with minimal direct supervision

12. Ability to react decisively and swiftly in emergency situations

Required Education and Experience

  • High School Diploma or Equivalent
  • Graduate of an accredited RN program

Required License/Certification

  • Valid State RN/LVN license
  • Current CPR certification

Preferred Education and Experience

  • A minimum of one (1) year of nursing experience in a SNF
