Details: POSITION OVERVIEW:The RN MAT Care Manager is a key member of the buprenorphine treatment team and serves an essential role in the implementation process of the program. This person must possess not only the clinical knowledge and skills to participate in individual patient treatment, but also the organizational and communication skills to execute program activities. The salary range is $90-110, 000/annuallyKey Functions:
- Provides nursing support and care management services to patients presenting for medication addiction treatment (MAT) including all duties related to patient care, such as referrals, patient education, coordinates medication delivery, ensures that required lab work is complete, and schedules/arranges follow-up appointments. Patient supports are provided pre-, during, and post-MAT treatment.
- In collaboration with other staff, provide orientation to buprenorphine treatment process with new participants and document each participant's treatment goals
- Participates in regular staff meetings focused on coordinating care within an interdisciplinary team, providing the team updates on patients' progress in care.
- Reinforce health education, which also includes overdose prevention education and response training, provided by the treatment prescriber related to buprenorphine treatment, medication adherence, overdose, HIV, and HEP C prevention.
- Respond to patient calls within a timely manner. Document all calls/visits; maintain accurate notes on telephone message and attach progress note in patient's chart. All documentation in a patient's medical record must be complete and easily understood by anyone who may need to access the record.
- Meet with MAT team and clinical staff on a regular basis and work jointly to establish expectations for how team issues will be managed, such as scheduling, forms, and refill issues.
- In case of patient referral to the Emergency Department or another facility, coordinate care with appropriate staff at the other facility (Mental Health Center, Emergency Department, other Community-Based Substance Abuse and Mental Health Treatment facilities etc).
- Assisting the prescribing physician in making referrals to community providers for counseling or higher levels of care when needed
- Maintaining therapeutic relationships with both the patient and the medical provider
- Adhere to required patient privacy standards and requirements such as HIPAA and 42 CFR part 2.
- . Record keeping that may be referenced for a DEA inspection
- Overseeing the following patient care components:
- -Care coordination including medication management and treatment monitoring
- -Insurance authorization and troubleshooting
- -Patient linkage to additional support when needed (drug treatment services and mental health care)
- - Facilitation of ancillary services (including patient transportation)
REQUIREMENTS:
- Bachelor of Science from an accredited institution[TJ3]
- Valid New York State Registered Nurse license and current registration
- One (1) year experience providing nursing services
- Previous relevant experience in delivering nursing services.
- Knowledge of agency's regulatory policies, procedures, and protocols[TJ4]
- Ability to deal with sensitive issues with diplomacy and discretion.
- Ability to use business acumen to exercise good judgment and decision-making.
- Experience working with Microsoft Office
- Experience using Electronic Medical Records (EMR)
- Bilingual Spanish/English, preferred.
