A home healthcare organization in New York City is looking to add a new, licensed Registered Nurse (RN) to join their growing Medical Respite Program in Manhattan. In this role, the Registered Nurse (RN) will be responsible for conducting psychiatric assessments, assessing physical health needs, and making appropriate referrals to Community Physicians. About the Opportunity: State Date: ASAPAssignment Length: ContractSetting: Medical Respite Program Schedule: Tuesday to Saturday Hours: 9am to 5pm (35 hours a week, 1 hr unpaid lunch) Patient Population: Homeless Responsibilities: The Registered Nurse (RN) will: Provide comprehensive primary health care, including case finding, health screening, health promotion, physical and psychosocial assessment, case management, and overall management of medications Develop, maintain, and revise written plans of care and general healthcare records Engage new clients in the continuum of care through outreach Collaborate with other health care providers and make appropriate referrals Serve as client advocate specifically in accessing the healthcare system Provide communication to team members and participate in community and apartment visits to clients Coordinate services with other community mental health services such as, inpatient, outpatient, emergency, or day treatment. Assist in the development of a program that closes gaps in the continuum of services Assist in the training of team members in the range of health promotion and illness prevention, treatment, rehabilitation, and support services available Act as a healthcare resource to clients and extended families Perform other duties, as needed Qualifications: Graduate of an accredited School for Nursing NYS Registered Nurse (RN) license Previous experience in Primary and Behavioral Health Intake Assessments Computer savvy Microsoft Office Suite proficient (Excel, Word, etc.) Proficiency with Health-related computer applications Solid assessment, clinical, and documentation skills Patient oriented Great interpersonal skills Excellent communication skills (written and verbal) Strong attention to detail Highly organized Desired Skills: 5+ years of experience working with Adults dealing with Senior Mental Illness Bilingual
