The Ambulatory Nurse utilizes a systematic approach to nursing by incorporating assessment, planning, implementation and evaluation in the practice of nursing. - -Provides healthcare to individuals, families, and communities within the RN scope of practice as defined by the Illinois Board of Nursing. -Main focus of the Medicare Wellness nurse will be to complete Medicare Annual Wellness visits with eligible patients; in addition, help with clinic nursing responsibilities as defined or needed.
- Ensures understanding of CMS Medicare Annual Wellness Visit requirements/visit workflows
- Provides and facilitates competent health care of patients and families in area of practice. Ensures adherence to established standards of care and scope of practice.
- Ensures -documentation of care is complete and accurate.
- Provides health and wellness education to individuals and groups.
- Acts -as preceptor and supports the development of other staff and healthcare students.
- Provides -leadership for initiatives designed to improve the quality of care or clinical outcomes.
- Assists -Nursing Supervisor/coordinator in the development of departmental standing orders.
- Works with Nursing Supervisor to ensure department is compliant with DNV standards.
- Perform telephone visits with eligible Annual Wellness Visit patients
- Complete -AWV as assigned per departmental benchmarks/guidelines
- Assist with coordination of care for patients age 18-150.
- Perform telephone triage of both acute & chronic needs for patients age 18-150.
- Assist with in office procedures, such as incision & drainage, joint injections.
- Administration of immunizations & medications to patients age 18-150 -
