DescriptionWhat We Are Looking ForJob Title: RN NavigatorSchedule: Monday - Friday 8:30am - 4:30pm Location: 800 Stony Brook Court Newburgh, NY 12550 Salary Range: $37.13 - $46.41 Salary Range is commensurate with experience. Job Summary:The RN Navigator is an experienced registered nurse who is accountable for the effective clinical care coordination of patients. As an RN Navigator, you will act as a liaison and educator to patients, families, and medical providers regarding the philosophy of hospice care. The RN navigator is responsible for providing a physical assessment, and admission to hospice as required.Hybrid position:This office-based position can be flexed to remote; decision to be made in conjunction with reporting supervisor. As warranted by the organization's needs, being present in the office can be required at any time.Essential Job Functions/Responsibilities:1. Maintain a hospice presence in the area hospitals.2. Acts as a partner in care with physicians, case managers, and other facility staff.3. Conducts Hospice informational meetings with patient/families and does full evaluations of Hospice appropriateness. Collaborates with Hospital MD, Hospital NP, Hospice Social Work Patient Navigators and Hospice IDG to identify the patient/family needs and develop the Plan of Care.4. Initiate and follow up on referrals from the assigned hospital to confirm appropriateness and timing of admissions. Ensure all necessary paperwork has been received by the Patient Access Department.5. Provides an initial physical assessment and admission to hospice as required based on the CMS guidelines.6. Works collaboratively to aid in the timely discharges and admissions of the referred patients by assisting with patient/family visits to ensure admissions are completed efficiently.7. Notify the hospital CM/SW and/or Discharge Planner of patient's pharmacy and coordinate DME needs with the Patient Access Department prior to discharge.8. Works diligently with hospital staff: case managers, discharge planners and other home care agencies to ensure timely care of potential Hospice patients/families.9. Confirms with the Finance Department patient's insurance coverage.10. Admits GIP level of care appropriate patients by following Medicare Guidelines and obtains necessary consents and authorizations to admit patient onto Hospice program. Provides follow up RN visits when patients are admitted GIP at the hospital.11. Facilitates physician/ chart conversion of patient from hospital to hospice status and notifies Hospice Patient Access Team and Finance department once the patient has been admitted.12. Establishes and maintains positive working relationships with current and potential referral sources.13. Ensures seamless transitions of patients to home care, nursing home or Hospice Residence by providing direct oversight of patient education and preparation for admission, Plan of Care initiation, and coordination of care with multiple service providers.14. Builds and monitors community and customer perceptions of Hospice as a high-quality provider of services.15. Provides leadership in strategic planning by identifying opportunities for additional or improved services to meet unmet customer needs.16. Maintains comprehensive working knowledge of Hospice contractual relationships and ensures that patients are admitted according to contract provisions.17. Completes necessary charting via Hospice computer database.18. Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Hospice.19. Maintains necessary education and medical requirements for Hospice, local hospitals, and SYMPLR as necessary.The above statements are intended to be a representative summary of the major duties and responsibilities performed by incumbents of this job. The incumbents may be requested to perform job-related tasks other than those stated in this description.RequirementsPosition Qualifications:1. Bachelor's Degree in Nursing required.2. Registered Nurse with a license in the state of New York.3. Five years' experience in health care management preferably in hospice or home care preferred4. Obtain CHPNA certificate within one year of hire.5. Ability to market appropriately and deal tactfully with customers and the community.6. Knowledge of criteria for Hospice services in various settings, governmental regulations, The Joint Commission standards, and private payor practices preferred.7. Demonstrates good communications, negotiation, and public relations skills.8. Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.Physical Requirements:1. Prolonged periods of sitting at a desk and working on a computer.2. Must be able to lift up to 30 pounds at times.3. Considerable reaching, stooping bending, kneeling, and/or crouching.4.
