Job Description:
This Night, Evening, and Weekend (NEWT) Crisis RN will be a versatile nurse with excellent assessment and nursing skills that translate to the bedside. The Crisis RN is responsible for delivering high-quality patient care through timely and responsible management of patient/family needs by responding to triage telephone calls, making visits as needed, and liaising with the medical team and vendors through the weekends and nights. Requirements•Consistently responds to HOPs Triage service via electronic and/or phone triage calls•Maintains log for calls and visits received•Makes home visits appropriately to assess patient needs, to provide nursing care needs and emotional support to family, as needed, when patient dies•Performs nursing evaluation visit to new admissions and determines eligibility based on agency criteria, as needed, in order to meet with patient and family members•Consults with patient's physician, Hospice staff, and vendors reporting changes in patient's condition•Observes and reports to physician and other personnel changes in patient's needs and condition which necessitate a change in the plan of treatment•Provides those services requiring specialized nursing skill in accordance with agency guidelines•Practices appropriate documentation in a timely manner, using clinical progress notes for nursing services delivered and completes verbal order form in a timely manner•Confers with supervisor on call for additional information and guidance, as needed, keeping supervisor informed of any problems that may challenge agency policy•Obtains report from weekend on call nurse to prevent any lapse in continuity of on call availability, relaying needed information the next morning to primary staff/supervisor•Maintains a current knowledge of the nursing profession by participation in formal education programs, conferences, workshops and professional organizations; maintains professional affiliations•Participates in implementing and supporting agency policies and procedures•Adheres to safety precautions to planning and making home visits•Participates in agency continuous Quality Improvement Program and Shared Governance•Attends monthly staff meetings, team meetings, and other meetings as requested•Provides guidance and direction to employees reporting to him/her. Ensures staff receives the needed training. Reviews and adjusts staff work duties as required•Proactively supports the Individual Achievement Review and Evaluation (IAR&E) performance evaluation process. Communicates with staff and provides frequent coaching and feedback to enhance individual performance•Follows HOP personnel policies and procedures (i.e., recruitment and hiring process, completion of accident reports, etc.)•Attends required meetings and participates in committees as requested•Maintains patient confidentiality•Completes other assignments and tasks as requestedEducation
  • Graduation from an accredited nursing school and a valid license as a RN in the Commonwealth of Virginia;
  • Two years relevant (you could add on or replace relevant with bedside, geriatric, long term care, onocology, cardiology, palliative, hospice, ICU, Step Down or any combination of the above) nursing experience required
  • The demonstrated ability to respond to the needs of patients and care givers in varied settings.
