Our client, one of the largest non-profit OPWDD organizations in New York, is looking for an RN Case Manager. This is a temp to perm role.Job Function:The RN Case Manager will oversee direct support professionals in a various home-care settings. Settings range from small houses to apartment complexes. Each Nurse Case Manager will be responsible for about 2 houses with about 5-10 patients in each, at most.This person will be expected to float between houses each day of the week as per required by patients' needs, but will only need to report to one home each day. They will not need to float among multiple house in a single day.This is a hands-off nursing position where the candidate will be directing DSP's on how to care for the patients and not doing the hands-on care taking work themselves.
