We are so glad you are interested in joining Sutter Health!Organization:SSC-Sutter Senior CarePosition Overview:This position serves as a primary support and liaison in monitoring and evaluating care of our participants in assisted living facilities also called retirement communities for the elderly and skilled nursing facilities to ensure patient safety and regulatory compliance. Responsible for communicating with the facility staff, participant and their family, and the center staff to facilitate care coordination, and serves as a resource by supporting the mission and vision of the organization.Job Description:EDUCATION:Graduate from an accredited school of nursingCERTIFICATION & LICENSURE:RN-Registered Nurse of CaliforniaBLS-Basic Life Support Healthcare ProviderAUTO-Automobile InsuranceDL-Valid Drivers LicenseTYPICAL EXPERIENCE: 5 years recent relevant experienceSKILLS AND KNOWLEDGE:Possess critical thinking skills, an in-depth knowledge of legislation guiding PACE and RCFE/SNF functionMust have the ability to function independently and as a member of an interdisciplinary team; ability to exercise independent, sound nursing judgment in planning and providing patient care.Ability to recognize the special needs of participants placed in facilities.Ability to communicate with patients and significant others in an effective, mature, caring manner.Ability to maintain harmonious constructive relationships with internal and external customers.Demonstrated effective written communication skills.Ability to be aware of surroundings and respond to emergency and/or urgent events. Ability to select and utilize patient transfer/transport equipment safely.Productivity standards - ability to meet productivity expectations.Must be detail-oriented.Ability to work in mentally and physically demanding environment.Must have demonstrated proficiency in Microsoft Office Suite, including Word, Power Point, and Excel. Ability to format tables, retrieve data via queries, and create presentations via Power Point. Ability to navigate an electronic health record.Ability to manage own time and schedule own tasks.
