Job Description:

Hudson Valley Hospice on-call nurse provides nursing care after office hours to Hudson Valley Hospice patients and their families. The nurse ensures that services are consistent with the philosophy, goals, objectives and policies of the Agency. ESSENTIAL FUNCTIONS

  • Provides services according to the patient/family care plan, in-home care, inpatient, and/or nursing home setting.
  • Makes visits to patients at home, in hospitals or nursing homes as needed or directed by the supervisor on-call.
  • Responsible for obtaining reports prior to the start of the on-call shift.
  • Provides accurate and complete on-call reports as required at the end of each on-call shift.
  • Carries Hudson Valley Hospice cell phone and laptop during scheduled work hours and responds according to policy and procedures.
  • Responds to all calls from service within 10 minutes of the initial page/call.
  • Provides palliative nursing care and symptom control in accordance with physician orders and IDT care plan.
  • Collaborates and communicates effectively with patients/family members, members of the Hospice Interdisciplinary Team, and other health care providers as appropriate across all care settings.
  • Provides education, supervision, and counseling to patients/family members and caregivers regarding physical care, observation, and return demonstration of procedures.
  • Documents all interventions, including phone calls in the EMR, as per policy and concludes with a plan of action regarding direction to specific staff, family, or patient as per EMR guidelines and policy.

  • Proficient use of computers including EMR's and Microsoft Word. Also, E-mail voice mail, copiers, fax machines, printers, and cell phones.
  • Demonstrates professional and technical competence to resolve identified issues.
  • Works effectively independently and cooperatively
  • Able to prioritize and handle multiple tasks efficiently.
WORK ENVIRONMENT/PHYSICAL DEMANDS The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
  • Possible exposure to highly contaminated agents.
  • Moderate to high risk of exposure to unsafe environments.
  • Routine duties involve the potential for direct exposure to blood or body fluids.
  • Regular travel by car in all weather conditions.
  • Works in patient homes and, on rare occasions, in hospitals and nursing homes.
  • Ability to carry up to twenty-five pounds of equipment and/or supplies.
PREFERRED EDUCATION & EXPERIENCE
  • Graduate of an accredited nursing school.
  • Valid New York State RN license
  • Three years of nursing experience including one year of medical-surgical experience preferred.
  • Experience in Hospice, community health and/or oncology nursing preferred.
  • Valid New York State driver's license.
  • Hospice and Palliative Care Certification or after completion of one year of employment is highly encouraged
  • Maintains a working knowledge re: use of the CADD Pump.
Hours: Full Time (40hr/week) - After Hour shifts - Evenings, Overnights and Weekends (4:30pm-12:30am, 12:30am-8:30am or 8:30am-4:40pm) Pay: $89,000- 95,000 annuallyFLSA: ExemptBenefits: Medical, Dental, Vision, Mileage, Company Matched 403b Retirement Plan and more!
