DescriptionLooking for a Rewarding Career?AuthoraCare Collective is currently seeking an RN On-call Weekend (Integrated Health Services, Home Health and Hospice). This is a Full-time position: Friday, Saturday and Sunday 10:00am - 8:30pm.Job Summary:Triages incoming calls for patients on Integrated Health Services. Assess and identify patient problems and respond appropriately with clinical intervention. Complete in-home clinical visits as applicable to assess and treat patient. Coordinate with appropriate resources to meet patient's after-hours needs. Work closely with members of the care teams to ensure quality communication and patient care. Schedule and add appropriate documentation to EMR with each patient interaction including obtaining physician orders. Complete assigned EMR workflow. Establish and maintain therapeutic relationship with patient, including providing disease education, medication management, clinical care and end of life care. Will send communication to appropriate discipline to ensure continuity of care. - Graduate of an accredited School of Nursing.
- Must be currently licensed as an RN in North Carolina and in good standings with NCBON.
- CPR Certification.
- One year of experience in an acute, hospice, home health setting, or long-term care setting. Two years of experience preferred.
- Bachelor's degree in nursing preferred.
- Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
Required Skills/Abilities:
- Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.
- Strong computer competencies needed including learning and using electronic medical record and documentation software.
- Ability to prioritize and clinically triage patients appropriately.
- Ability to act with integrity, professionalism, and confidentiality.
