License: NYS RN/LPN License Certifications: BLS Certification Must-Have: EPIC experience highly preferred; Japanese speaking preferred but not required Job Description: - Deliver patient care by incorporating the tenets of Relationship Centered Care through nursing processes based on theory, research, evidence-based practice, and organizational nursing standards.- Assess and evaluate patient care needs using critical thinking skills in patient care management.- Integrate relevant assessment and intervention skills in nursing care delivery.- Collaborate with the interdisciplinary healthcare team in the development, implementation, and evaluation of care plans.- Prioritize all aspects of patient care, including teaching, rounding, coaching, planning post-hospital care, and appropriate delegation.- Educate patients and caregivers on care plans, transition of care, health promotion, and disease prevention, while considering their readiness to learn.- Communicate effectively and professionally with patients, families, and all members of the Interdisciplinary Patient Care Team.- Manage assignments within the Care Delivery Model of Modified Primary Nursing and demonstrate effective allocation of material resources.- Role model Relationship Centered Care by facilitating actions such as Admission Welcome, Bedside Shift Report, and Purposeful Hourly Rounding.- Establish and maintain therapeutic relationships with patients and families while considering their values, preferences, cultural diversity, expressed needs, and knowledge in all aspects of care.- Use evidence-based practices to enhance understanding of patients' perceptions of care and initiate service recovery for patients and families with concerns.- Maintain patient-centered care practices, identifying and escalating opportunities for improvement and safety barriers.- Collaborate with interdisciplinary teams for quality improvement projects, evidence-based practice activities, and nursing research studies.- Perform charge nurse duties and coordinate with nursing management to ensure efficient use of resources and supplies.- Contribute to the environment of care, supporting clinical colleagues, patients, and healthcare team members.- Project a professional image and communicate with styles and methods that demonstrate care, respect, compassion, and empathy.- Participate in shared decision-making through specialty practice councils, nursing department committees, and unit initiatives. Education: Bachelor of Science with a major in nursing preferred. An associate degree in nursing is considered with relevant experience and/or active matriculation in a BSN Program.
