DescriptionGENERAL SUMMARY: Under the supervision of the Director of Admissions, the Outreach RN is responsible for referral development and facilitating admissions for inpatient rehabilitation, skilled nursing and home health. Referral development involves establishing professional contacts as referral sources and marketing programs and services to these contacts. Facilitating admissions involves the clinical evaluation of referrals, completion of required paperwork, and marketing services to referred patients/residents. The Outreach RN must be able to demonstrate the knowledge and skills necessary to evaluate the age-specific clinical needs of potential admissions to the Allied Services health care system, and determine their appropriateness for admission to Allied Services' various levels of care. Type of supervision exercised: none. Jobs reporting to this position: none. EDUCATION: Graduate of an accredited Nursing Program required. CRRN preferred. WORK EXPERIENCE: Minimum of five (5) years experience as a registered nurse required. Rehabilitation Nursing, Skilled Nursing, and Home Health experience preferred. SPECIAL EMPLOYMENT REQUIREMENTS: Possession of a current license as a Registered Nurse as issued by the Pennsylvania Board of Nurse Examiners required. Must be cleared by PA State Police criminal history check. Valid PA driver's license required for travel to referrals sources. Good written and verbal communication skills and interpersonal skills required for effective communication with administration, staff, residents, families, physicians, and representatives of other external customers. Must have knowledge of admission criteria for inpatient rehab facilities and skilled nursing facilities. Must maintain confidentiality of all contacts with residents/families. PHYSICAL DEMANDS: Sedentary Work: duties require exerting up to 10 lbs. of force occasionally (approx 10-33% of shift) and/or a negligible amount of force frequently (approx 33-66% of the time) to lift, carry, push, pull, or otherwise move articles such as files. Duties require the ability to transport self and health information throughout referring facilities. Walking, standing, moving within referring facilities comprise most of this positions work day. Near visual acuity must be accurate to read and interpret reports, data, etc. ACCIDENT OR HEALTH HAZARDS: Occasional exposure to blood and body substances. Exposure to needle sticks possible. WORKING CONDITIONS: Works in a well-lit nursing unit, which includes: nurses station, patient's rooms, treatment areas, physician offices, etc.
