Responsibilities Visits with assigned caseload of patients and families in their homes to complete guided assessments, support medication management, address complex symptoms and coordinate care. Engages patients and caregivers in discussions about their goals of care, review advance directives and collaboratively develop treatment plans based on goals. Monitors care plans in conjunction with the rest of the clinical care team. Advocates for the patient and caregivers in liaison with other care providers to ensure care is coordinated and consistent with patient's goals. Participates in the measurement and assessment of care-related processes and outcomes to improve the quality of patient care. Communicates plans of care to primary physicians and other consulting clinicians. Educates patients and family on trajectory of illness, purpose of medications (also reviews side effects and how to administer) and assesses disease understanding. Performs other duties as assigned. Qualifications Associate degree in nursing (bachelor's preferred). Current RN license, specific to the state(s) you are assigned to work.
