We've been providing compassionate, patient and family-centered care to the people of our community during and after the last season of life since 1982.Hospice of Lenawee is hiring for a: RN - Part-time - Afterhours - Hospice - Home CarePart-time is 24 hours per week with a weekend rotation and a holiday rotation.$33/hourWorking with our AMAZING team = PRICELESS!The Afterhours Home Care RN works on-call overnight and is contacted via cell phone if needed to make a visit to either a facility or a patient's home after the patient has been triaged. We're looking for a Michigan licensed RN (hospice experience preferred but we are willing to train) who brings excellence, compassion, and integrity to the end-of-life journey.   • The Night & Weekend RN supports the mission of Hospice of Lenawee by providing care to individuals and families based upon the philosophy, policies and procedures of Hospice of Lenawee. The Night and Weekend RN uses the nursing process of assessment, problem identification, planning, intervention, and evaluation.Essential Job Responsibilities of a Hospice RN:
    • Provides skilled nursing care based on a compassionate and holistic approach.
    • Advocates for pain/symptom control.
    • Teaches, supervises, and counsels the patient and family regarding care concerns.
    • Maintains close communication and involvement with the attending physician, home health agency, spiritual advisors, and other persons that may be participating in the care.
    • Performs admission of patient to hospice care when needed.
    • Provides nursing care and supports Hospice Home staff when needed.
    • Responsible for departing from location within 15 minutes of receiving notification of patient need. If unable, utilizes Back Up Nurse and Supervisor on Call, when appropriate.
    • Audit clinical records for compliance with Hospice of Lenawee policies and procedures and report findings to QAPI Coordinator.
    • Meets safety needs of patients and uses equipment safely and appropriately.
    • Documents and records care provided according to agency standards.
    • Responsible for learning and utilizing technology made available by the agency to increase efficiency, communication, and performance of duties.
    • Be knowledgeable of and follow policies and procedures set by Hospice of Lenawee.
    • Holds all patient information in strictest confidence and discloses information and data only to persons authorized by Hospice of Lenawee.
    • Performs other duties and responsibilities as determined appropriate by supervisory staff and/or management.
    • Demonstrates willingness and flexibility to work different and/or extended shifts when needed.
    • Presents appropriate professional appearance in accordance with Hospice of Lenawee policies.
    • Participates in weekend and Holiday rotation.
    • Hospice of Lenawee direct care staff may have exposure to infectious diseases. Must comply with standard precautions to prevent the spread of infectious disease.
    • Attends staff meetings and completes all mandatory training.
    • Complies with attendance policy and is physically at assigned location to fulfill job responsibilities and functions.
    Qualifications of a Hospice RN:Required Knowledge, Skills, and Abilities of a Hospice RN:
    • Familiarity with community health organizations, practices and resources.
    • Familiarity with the Hospice philosophy of care for the dying and in agreement with the principles of HOL care and a commitment to it.
    • Demonstrates ability to make sound clinical judgments and act upon them in a timely manner.
    • Demonstrates general technical abilities with nursing functions, such as, IV administration, equipment function and maintenance.
    • Ability to plan, assess, implement and evaluate nursing care for Hospice of Lenawee patient/families.
    • Ability to function autonomously in a home setting.
    • Demonstrates excellent customer service, both internally and externally.
    • Possesses strong communication skills and the ability to interact positively and professionally with patients, families, and other team members.
    • Demonstrates ability to function as a team member, collaborate with coworkers on different shifts, positions, and departments to promote teamwork and inclusion.
    • Demonstrates interest, compassion, and empathy for the terminally ill and is comfortable with issues of death and dying.
    • Is self-aware and asks for help when the need arises.
    • Demonstrates ability to read, write, and follow direction and instruction.
    • Has the physical ability necessary for extensive standing, lifting, and pushing.
    • Demonstrates ability to prioritize and manage time effectively.
    Education/Credentials of a Hospice RN:
    • Current Michigan Registered Nurse License.
    • BSN degree, preferred.
    Experience of a Hospice RN:
    • Two years nursing experience in hospice, home health, or oncology nursing, preferred.
    • Experience with dying patients and their families, hospice experience preferred.
    • Experience in health care, required.
    Additional Requirements of a Hospice RN:
    • Valid driver license and automobile with current insurance.
    • Working cell phone.
    Position Status/Classification: Non-exempt, Regular, Stipend for Availability, Patient Care
