The Home Care RN uses the nursing process of assessment, problem identification, planning, intervention, and evaluation. Essential Job Responsibilities of an RN:
  • Provides skilled nursing care based on a compassionate and holistic approach.
  • Updates a comprehensive care plan that addresses physical, emotional, sociological, and spiritual needs of the patient/family in conjunction with the patient/family and program staff.
  • Advocates for pain/symptom control.
  • Provides telephone triage support to Clinical Team.
  • Teaches, supervises, and counsels the patient and family regarding care concerns.
  • Directs and supervises LPNs and Hospice Aides, as requested.
  • Maintains close communication and involvement with the attending physician and IDT members involved in the patient's care.
  • Performs admission of patient to hospice care when needed.
Qualifications of an RN:Required Knowledge, Skills, and Abilities of an RN:
  • Familiarity with community health organizations, practices, and resources.
  • Familiarity with the Hospice philosophy of care for the dying and in agreement with the principles of HOL care and a commitment to it.
  • Demonstrates ability to make sound clinical judgments and act upon them in a timely manner.
  • Demonstrates general technical abilities with nursing functions, such as, IV administration, equipment function and maintenance.
  • Ability to plan, assess, implement, and evaluate nursing care for Hospice of Lenawee patient/families.
  • Ability to function autonomously in a home setting.
Education/Credentials of an RN:
  • Graduate of approved School of Nursing.
  • Current Michigan Registered Nurse License.
  • BSN degree, preferred.
  • CPR certification, required.
Experience of an RN:
  • Two years nursing experience in hospice, home health, or oncology nursing, preferred.
  • Experience with dying patients and their families, hospice experience preferred.
  • Experience in health care, required.
Additional Requirements of an RN:
  • Valid driver license and automobile with current insurance.
