Bonus amount prorated for Part time hires, per diem hires are ineligible. o Current MaineHealth member employees are ineligible; former MaineHealth Members are ineligible until greater than 6 months separation from employment. This is a full-time, 36 hour per week position. Your role: Pen Bay Family Medicine | MaineHealth is located at the Beebe Health Center in Rockport, Maine and is looking for an RN Patient Care Coordinator. Under the direction of the PBPA Clinical Care Manager, the RN Patient Care Coordinator uses professional nursing skills to coordinate the health care needs of patients in the practice identified as at risk due to chronic or acute illnesses or those in need of preventative health services. The RN Patient Care Coordinator works with the provider, practice care team, patient and family to promote self-care management and provides direct care appropriate to the role of the nurse including patient education, motivational interviewing and development of self-care action plans.Required Minimum Knowledge, Skills, and Abilities (KSAs):
  • Education: Graduate of an accredited School of Nursing required; Bachelor's Degree preferred.
  • License/Certifications: Current applicable state(s) license as a Registered Professional Nurse required.
  • Experience: Two years of RN experience required. Practice or ambulatory clinic setting preferred.
