Overview:
We're looking for a Patient Care Manager to join our team. You will report directly to the Executive Director or Administrator. You will be responsible for day to day operations of your assigned home health site, administering the clinical aspects of the home health program and for ensuring the provision of quality of care to patients. The role is responsible for facilitating the relationship between referral sources, patients, caregivers, and employees. Depending on branch census you may be responsible for other duties as assigned.
- Ensure that the client's plan of care is individualized and appropriate, executed as written and reassessed by the appropriate health care processional when there is a significant health status change in the client's condition, at the physician's request and after hospital discharge.
- Ensure appropriate documentation is completed for all patients transferred to an inpatient facility.
- Manage the site in the absence of the Executive Director or Administrator.
- Managing and overseeing clinical and administrative and recordkeeping functions.
- Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients homes.
About You:
- Associate's degree in Nursing from an accredited school of nursing. Unless superseded by requirements listed in the state-specific section.
- Bachelor degree in nursing form an accredited school of nursing preferred.
- Minimum three years experience in direct patient care in a clinical setting. Unless superseded by requirements listed in the state-specific section.
- Must be able to effectively operate nursing assessment tools and office equipment to include, computer and or tablet.
- Current RN licensure in state of practice
- Current CPR certification
