Description$10,000 Sign on Bonus. (Prorated for part time hires) Responsible for nursing assessment of clients in the home in the Greenfield area. Developing a plan of care (POC) with the physician, client, family, updating the POC, and changing the POC as needed by ongoing intermittent assessment and evaluation. Plans for coordinated nursing service for individuals and families. Appraises individual and family health needs and hazards (existing or potential). Provides intermittent nursing care to clients as well as teaching and supervision. Consults with and refers families to appropriate personnel within the Agency or other community resources.RequirementsEducation: Graduate of a N.L.N. approved school of nursing.Experience: One-year nursing experience in an acute care setting or comparable experience is recommended. Experience in community health nursing preferred.Licensure: Valid, unencumbered license in the State of Massachusetts. Possession of a valid driver's license and dependable, registered transportation Other: Basic knowledge of computer use, adequate typing skills and spelling skills. Ability to read, write, speak and understand the English language.
