Job Summary:

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our dynamic healthcare team. The ideal candidate will provide high-quality nursing care to patients in an assisted living facility. The RN will be responsible for assessing patient needs, implementing care plans, and ensuring the highest standards of patient care in accordance with regulatory guidelines and best practices.

Key Responsibilities:

• Supervision of LPN staff
• Coordination of Care of the patients at assigned facility
• Updating and Implementing MD orders
• Field based Nursing Work (Oasis)
• Completion of SOC/Recertification/ ROC documentation
• Coordinate resources & services for assigned caseload, working with the patient and physician
• Must be pro-active and take initiative when dealing with the patients
• Must provide quality and timely documentation after providing care
• Maintain up to date/accurate documentation for assigned cases
• Maintains a cooperative relationship among health care teams

Qualifications:

- Education: Bachelors degree in nursing (BSN) or Associate degree in Nursing (ADN) from an accredited program.

- Licensure: Valid Registered Nurse (RN) license in the state of New York.

- Skills: Strong clinical assessment skills, critical thinking abilities, and excellent communication skills.

Physical Requirements:

- Must be able to lift and reposition patients, as well as move various equipment, while adhering to safety guidelines. - Ability to stand, walk, bend, and perform other physical activities for extended periods.
