This recruitment offers a $10,000 Signing Bonus to eligible new employees joining UW MedicineHarborview Medical Center has an outstanding opportunity for a Registered Nurse (RN) to join our Senior Care Clinic.WORK SCHEDULE:100% FTE8 hr Days, M-FDEPARTMENT DESCRIPTION:The Senior Care Clinic at Harborview provides primary care and consultative services for patients aged 65 years and older who also have a geriatric syndrome such as cognitive impairment, falls and gait instability, frailty or functional decline.A compassionate, multidisciplinary team provides a range of services including geriatric medicine, psychiatry, pharmacy, nutrition, social work and nursing.The clinic provides consultative care and referrals to other disciplines as needed for patients concerned about osteoporosis and other skeletal health issues. Our mental healthcare professionals provide a range of services including psychiatric consultations, medication management and recommendations for continued psychiatric care. Health education is available also through the Harborview Patient and Family Resource Center.POSITION HIGHLIGHTS:Conveniently located near public transportationMake a difference for the patients and employees we serveOpportunity to provide career growthPRIMARY JOB RESPONSIBILITIES:In-person and phone triage, in basket management, performing thorough symptom assessment, evaluating level of acuity, developing clinical interventions and plans of care, and care managementTransitions of care and interacting with home health and community living providers (adult family homes, assisted living facilities, and memory care units). Problems addressed during nurse visits include blood pressure management, edema management, foley management, wound care, IV starts, one-on-one teaching about safety issues relevant to seniors such as falls assessment/prevention and medication administrationREQUIREMENTS:Active WA State or Multi-State RN License by start dateAt least 2 years of professional acute care, med surg, or primary care nursing experienceABOUT HARBORVIEW MEDICAL CENTERAs the region's only Level I Trauma and verified burn center, Harborview Medical Center is a comprehensive healthcare facility owned by King County and operated by UW Medicine.
