This is an every weekend position 12 hour shifts both Saturday and Sunday and one 7.5 hour shift during the week.

Job Details:

Our Home Care RNs:

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
• Measure vital signs, Administer IVs, change dressings, clean wounds as needed
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
• Provide patient and family education for disease management and health promotion as directed by physicians plan of treatment.
• Provide care to the most upmost standard

Minimum Qualifications:

• New York State RN license
• Minimum of one year of recent hands on experience as a Registered Nurse.
• Basic Life Support Certification
• Valid New York State Drivers License and reliable transportation

Posted Salary Range:USD $65.00 - USD $72.00 /Hr. :
