$6,000 sign-on bonus, Exceptional Weekend Shift DifferentialEast Pasco - Sat/Sun 8a-12a (32 hour work week) - Reporting to the Clinical Care Manager, the Hospice RN Weekends is responsible for providing quality care to patients with GHS. The Field includes patients who reside in private residences, assisted living facilities, and skilled nursing homes. The Hospice RN Weekends utilizes assessment, intervention, and education to maximize patient comfort and to enhance the quality of life for the patient/family. The Hospice RN Weekends oversees and case manages the needs of the patients assigned to them.Education and Qualifications: Currently licensed in the State of Florida as a registered nurseMinimum of one (1) years nursing experience in a hospital, home health or hospice setting preferredStrong ethical standards and an appropriate professional demeanorShould possess sound critical thinking, time management, technical skills, analytical ability, good judgment, and strongThe ideal candidate is a well-organized and self-directed individual who is a team playerMust have reliable transportation to perform tasks and responsibilities in a timely and appropriate fashion. Must be able to provide proof of automobile insurance and have a valid Florida driver's license, a copy of which will be placed in their employee file. Essential Job Responsibilities: Case manages the needs of the patients and families in the field assigned to themPerforms ongoing assessment of physical, psychological, social, and spiritual needs of the patient/family unit and initiates appropriate interventionProvides and/or oversees supervision of P.N.'s, C.N.A.'s, and other assigned staffResponsible for completing admissions and hospital referrals asSupport and assist hospice inpatient units or crisis care cases asAssist with audits or other duties as specified by CCM during downTakes independent action to identify problemsSupports policies and proceduresAssists in the orientation of new staff membersFunctions as a resource person; provides mentoring and guidance for co-workers and additional staffMaintains positive working relationships with team members of all disciplinesAttends to patient/family complaints and concernsCreates and promotes initiatives to develop and strengthen relationships with facility partnersTakes a proactive approach to identifying and resolving potential issuesEducates the community regarding hospice care, philosophy, and concept through individual consultation with persons in the community,Communicates regularly with the Clinical Care Manager and continuously assesses areas for risk managementMaintains current with organizational and departmental communication via e-mail. All nursing staff are required to check their e-mail accounts at the start of each shift.Effectively communicates, collaborates, and coordinates with colleagues and management on issues related to nursing process and serviceCreates and implements nursing care plans which are reviewed daily and updated as neededConsiders financial implications in rendering cost-effective nursing careEnsures professional standards of nursing practice are maintainedCompletes required documentation prior to departing from scheduled shiftActively participates in the IDG (Interdisciplinary Group) and communicates patient needs appropriately to other disciplines when requestedCollaborates with team physician daily on the status of the patientPerforms other duties as requested by the Clinical Care Manager #GHSJobs
