Sapphire Center for Rehabilitation is seeking a RN Wound Care Specialist for its skilled nursing facility located in Queens County, New York. In this role, the successful candidate will use his/her knowledge, skill and experience to plan, coordinate, implement and evaluate programs for all patients that require wound care initiatives.

Job Responsibilities:

v Collects, and analyzes process and outcomes data related to nursing skin and wound care utilizing internal and external metrics and benchmarks. Identifies opportunities and strategies for performance improvement.

v Monitor and assess any new skin observation alerts, recommend treatment.

v Participates in the development, evaluation and revision of standards of patient care based upon current national guidelines. Collaborates with leadership and multidisciplinary teams to establish protocols which support best practices to achieve optimal clinical outcomes. Directs organization-wide initiatives for skin and pressure injury prevention, treatment and care.

v Serves as a resource for wound, ostomy and continence care. Provides consultation and direct assistance to the care team on issues related to patients with actual or potential alterations in skin integrity.

v Assists the nursing and medical staff in maintaining current knowledge through formal and informal educational offerings. Conducts formal orientation of new nursing and ancillary support staff.

v Develop wound care program in partnership with the Director of Nursing and consultant Physician.

v Develop nursing care plans for resident at risk or currently with skin integrity issues, include treatment protocols.

v Provide resident and staff education. Work collaboratively with Physical therapy and nursing staff to maintain skin care programs.

v Supervise and direct the activities of nursing personnel to ensure the provision of wound care to the residents of facility.

v Monitor the effectiveness of the plan of care through direct observations of care rendered and through weekly wound rounds/ reassessment.

v Develop plan of care for resident with a current pressure sore, or evaluated as high risk for the development of pressure sores.

v Provides reports on the type and frequency of wound in the facility.

Required Qualifications, Skills and Abilities

Education: Completion of a formal education program in Nursing

License/Certification: Valid NYS RN License, CPR certified

Equipment/Computer Software: Basic computer/ EMR software
