The Wound Care Nurse, RN works closely with the Wound Care Coordinator team consisting of LPNS, CRNP, the Nurse Practitioner and interdisciplinary care team. The Wound Care Nurse collaborates with the Wound Care team to determine appropriate wound prevention and treatment. Implements treatment modalities as assigned.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Checks skin condition on new admissions using standardized tool.
Completes Braden Scale as needed.
Obtains physicians orders for treatment modalities as determined in collaboration with the Wound Care Team.
Implements treatment plan on residents as appropriate and administers treatment.
Appropriately documents on residents receiving treatments, reviewing previous documentation and assuring consistency in documentation.
Completes Initial/Weekly Skin Impairment form.
Works with Restorative Nursing to make sure staff is following through with active measures to prevent skin breakdown and that all residents who are at risk for skin breakdown are identified.
Works with Nursing Staff to ensure that all residents have appropriate pressure redistribution surfaces and devices.
Updates care plans as needed for residents with skin impairment.
Problem solves with staff to resolve problems that arise in preventing and treating skin impairment.
Works closely with Physical and Occupational Therapy, Central Supply and Vendors to ensure proper use of pressure redistribution surfaces and devices.
Informs residents and responsible parties of any changes in skin integrity, deterioration of wound, changes in care and treatment of wound.
Reports relevant information to the Direcctor of Nursing, Unit Manager/ RN Supervisor including change in any residents skin status, deterioration of a wound, or failure of an individual staff to provide appropriate pressure reducing measures.
Attends in-service education programs as assigned and applies information to job tasks.
Education/Training/Work Experience:
Education: Graduate of accredited School of Nursing
Certificate/Licenses: Current, active license as a Registered Nurse in Pennsylvania. Wound Care Certification preferred.
Work Experience: Minimum of two years experience providing direct care to residents in a long term care setting.
