Role Description This is a full-time on-site role for a RN Hospice Case Manager in Edgewater, NJ. The RN Hospice Case Manager will be responsible for discharge planning, utilization management, nursing, medicine, and case management. Qualifications Discharge Planning and Utilization Management skills Nursing and Medicine knowledge Experience in case management Excellent communication and interpersonal skills Ability to work in a team and independently Bachelor's degree in Nursing or related field Experience in hospice care is a plus Current RN license in the state of New Jersey
