Responsible for case management intake assessments and performing comprehensive assessment of health care needs.Duties:* Fulfill nurse protocol training requirements in HIV, STD, Immunization, and other relevant areas to function as an expanded role PHN.* Provide direct nursing care including physical assessments, education, treatment, and develop and document a plan of care consistent with the clients' level of need.* Screen and refer clients to other community service providers as needed. Assist with referrals and follow-up treatment planning as indicated.* Provide sexual health promotion and prevention education, including counseling clients and family regarding the importance of medication adherence for viral suppression.* Participate in staff meetings and assist with developing and implementing quality improvement projects as needed.* Maintain accurate medical records, documenting is a must, assist with data entry and quality assurance monitoring.PLEASE DO NOT APPLY ON THIS SITE.
