As a Clinical Coordinator, you'll play a crucial role in assessing and addressing clinical issues that affect residents' success in supportive housing. - Provide intensive in-home counseling and/or linkage to referrals for a small caseload of residents living in Bridge housing throughout the Bronx and Manhattan.
- Work collaboratively with internal departments and external providers to coordinate care.
- Provide clinical direction and guidance to supportive housing staff.
- Promote physical and mental health, safety, well-being, and recovery with clients.
QualificationsRequired Knowledge, Skills, and Abilities:
- A Master's degree in Social Work or Mental Health Counseling.
- Two years of experience working with individuals facing substance abuse and mental health issues.
- Strong computer literacy required; proficiency in using Electronic Health Record (EHR) systems to document services.
- Knowledge of AWARDS, CARES, and BCS systems is advantageous.
- Potential for clinical supervision to accumulate LCSW/LMHC hours, contingent on staff availability.
- Provided with an unlimited monthly MetroCard for commuting needs.
- Bilingual proficiency in Spanish is a plus.
