Founded as a result of the federal Older Americans Act, the AAAs are mandated to facilitate the area-wide development and implementation of a comprehensive, coordinated system for providing long-term care in home and community-based settings, in a manner responsive to the needs and preferences of older individuals and their family caregivers. Requirements Description of Role: Reporting to the Scheduling Support Supervisor, the Scheduling Coordinator is responsible for scheduling, rescheduling, and managing all cancellations of assessment visits for the LTCA Assessment Workers. The Scheduling Coordinator also assists with assigning and reassigning cases in the Pennsylvania Individualized Assessment, (PIA) data base to Assessment Workers. These tasks require an understanding of Long-Term Care programs and the ability to explain the assessment process to Consumers and referral sources. Required Education and Experience:
  • Two years of Business College, Associate Degree, or the equivalent of experience and education.
  • Experience in social services and computer applications preferred.
  • Preference for individuals with knowledge of Scheduling and coordinating appointments for consumer/ customers.
Job Responsibilities: a. From the Scheduling Queue in LTCA's Management System (LMS), the Coordinator will select cases to schedule and meet the expected daily quota for calls. b. Complete the 1-2 call procedure within 48 hours for each case. c. Close cases timely in all electronic systems within 5 business days after no response from sending an unable to reach letter. d. LTCA Management Tracking will be updated for each case and the type of case will be selected when scheduling an assessment visit. e. Assign scheduled cases to Assessors in the Pennsylvania Individualized Assessment system, (PIA). f. Coordinate and arrange for interpreters with the contracted interpreter agency. g. Ensure each Assessment Worker has the proper weekly quota in their schedule. h. Fill cancellations as they occur from the scheduling Queue. i. Coordinate same day assessment case coverage as needed by un-assigning cases and reassigning them to available Assessment Workers. j. Close cases in PIA using appropriate set closure reasons. k. Sends daily unable to schedule messages to the Scheduling Supervisor. l. Maintains confidentiality of all consumer records per HIPPA. m. Models enthusiasm for PCA and LTCA's mission. n. Other duties as assigned. Professional Characteristics:

  • Ability to quickly acquire necessary knowledge and manage a high volume of telephone contacts.
  • High level of flexibility.
  • Ability to resolve scheduling conflicts.
