Community Care, Inc., a leading provider of home health care services is seeking a detailed-oriented and highly organized Scheduling Coordinator to join our team. The Scheduling Coordinator pays a crucial role in ensuring the seamless delivery of care services to our patients by efficiently coordinating schedules for our dedicated team of home health professionals.
Key Resposibilities:
- Create and manage schedules for home health aides, nurses and other team members to ensure all patient visits are properly staffed and scheduled.
- Match the skills and qualifications of our team members with the specific needs and preferences of our patients.
- Communicate with patients and their families to coordinate visit times and ensure their satisfaction with our services.
- Handle last minute schedule changes, call-offs and emergencies with a focus on maintaining continuity of care.
- Collaborate with clinical staff to ensure that care plans and schedules align with patients' healthcare needs.
- Maintain accurate and up-to-date scheduling records and patient information in our system.
- Assist with recruitment and onboarding of new team members.
- Maintain compliance with all applicable laws, regulations and company policies.
Qualifications:
- High school diploma or equivalent (Bachelor's degree preferred)
- Previous experience in healthcare scheduling or a related field is highly desirable.
- Strong organizational skills with excellent attention to detail.
- Exceptional communication and interpersonal skills.
- Proficiency in computer software, including electronic medical records.
- Ability to work under pressure and handle scheduling conflicts with diplomacy and efficiency.
- Knowledge of home health care regulations and processes is a plus.

- Ability to work independently and as part of a team.
